- Letter of Conduct from Institution
- Teacher recommendation (2 minimum)
- 1 photo 2X2
- Birth Certificate
- COVID-19 Vaccination Certificate
- Medical Certificate
- Social Security Card
- Credit certification from previous institution
- Psychometric Evaluation (Pre K and K )
- Psychoeducational Evaluation (1st - 11th grade)
- Learn Aid, PIENSE, PSAT, CAREME test score
- Attendance and punctuality report
